Support for Children
and Young People
Expert NDIS support services to empower young people and help them build a successful foundation for their future.
Person-centred, trauma-informed care.
Real growth happens when children and young people feel secure and truly understood.
At Plan and Grow, we provide person-centred, trauma-informed support designed to turn individual goals into a lived reality. We walk alongside participants to navigate NDIS plans, ensuring every step taken is a meaningful move toward a self-determined life and a brighter future.
Every young person deserves a strong start to their next big chapter. Through tailored community services, we provide the reliable framework children and young people need to navigate life’s transitions with confidence. We aren't just a service; we create a stable foundation for them to thrive and achieve their own version of success:
Better care, better services.
High-quality NDIS support begins with a commitment to safety. And we are dedicated to providing just that—the secure foundation every young person needs to confidently navigate their next big chapter. By adhering to the Better Care, Better Services framework and rigorous quality standards set out by the Children and Community Services Act 2004, we ensure our organisations provide a gold standard of support. This fosters a secure environment where young NDIS participants can grow with dignity and realise their full potential.
The key principles that guide our care
A Foundation of Stability
Children and young people receive consistent NDIS support that prioritises their need for a stable and predictable care. Plan and Grow provides professional supported services to ensure a safe, home-like environment throughout their NDIS plans.
Leading the Way
Young participants and their loved ones are continually engaged to lead the decision-making process for their NDIS plans. This active involvement empowers young people to take direct ownership of their future.
Cultural Connection
Aboriginal children and young people are empowered by our support networks to maintain meaningful connections to their family, community, and land. We integrate the Aboriginal and Torres Strait Islander Child Placement Principle into every NDIS plan to protect their heritage and support their future.
Understanding
Every Need
Our team utilises individualised assessments to ensure NDIS support is perfectly aligned with each participant's specific goals. This child-focused practice ensures that young people receive the most effective supported services for their future.
Nurturing Identity
Children and young people flourish when they feel a sense of belonging. We empower them to strengthen their bonds with family, culture, spiritual sources, and communities. This support helps them develop their identity with confidence and realise their full potential in a safe, supportive environment.
Preparing for Independence
Young people receive high-quality NDIS support to prepare for the transition out of care and into adulthood. We provide the necessary supported services to ensure participants are adequately equipped to take charge of their future.
A Voice for the Future
Young people are empowered through their NDIS support to understand their rights and confidently express their needs. We ensure that every concern is heard and resolved promptly to safeguard their future.
Expert, Compassionate Care
Children and young people are supported by a team of highly trained professionals who provide safe and consistent NDIS support. We ensure every staff member is equipped to deliver exceptional supported services that contribute to a brighter future.
Integrity and Focus
Children and young people are the central priority in every organisational decision and action we take. We remain fully accountable to ensure all supported services are high-quality and dedicated to the best outcomes.
